<h3 class="wp-block-heading">Sub: Revised terms and condition for Senior Residents working in Railway Hospitals.</h3>



<p>Ref: Board’s letter No.88/H/2-1/9 dated 24.06.2008</p>



<p>Terms &amp; conditions for engagement of Senior Resident in Railway Hospitals have been issued vide Board’s letter cited under reference. Leave entitlement for Senior Residents as mentioned in the letter is reproduced below:</p>



<ul class="wp-block-list">
<li>One day weekly off by rotation.</li>



<li>Entitles to total 20 days leave in one contract year which can be availed as not more than 10 in six months period.</li>



<li>Not entitled to any vacation or any other type of leave.</li>
</ul>



<p>Now, it has been decided, with the approval of the competent authority in Railway Board that Senior Residents will be entitled for Maternity leave of 26 weeks of which not more than eight weeks shall precede the date of expected delivery OR remaining tenure of contract period, whichever is less. This is subject to her having completed working period of not less than 80 days in the twelve months immediately preceding the date of her expected delivery in the Railway hospital and fulfilling the other terms and conditions of Maternity Act, 1961.</p>



<p>This issues with the concurrence of Finance (Establishment) Directorate of the Ministry of Railways.</p>

<h3 class="wp-block-heading">Sub: PROPOSAL OF MEDICAL INSURANCE SCHEME FOR RAILWAY EMPLOYEES.</h3>



<p>Ref: 1) Railway Board letter No.E(W)2020/Misc/ Dashboard-GIS, dated 27.10.2020<br />2) Railway Board order No.ERB-1I/2020/23/30, dated 20.10.2020.</p>



<p>1) Ministry of Railways constituted a committee to examine and tender their recommendations to bring all Railway Employees under the Medical Insurance Scheme for obtaining treatment in private hospitals. IRTSA submits the following for the kind consideration of Railway Board,</p>



<p>2) <strong>Indian Railways Medical Service (IRMS) </strong>is spread across the length and breadth of the country. IRMS is having 125 hospitals and 586 dispensaries, manned by 33,300 medical staff catering to approximately 66 lakh beneficiaries including 12.27 lakh serving employees. In the year 2018-19 Railway Hospitals treated 2.01 crore OPD patients, 5.08 lakh in-patients and performed 1.02 lakh major and specialty surgeries.</p>



<p><strong><a href="https://centralgovernmentnews.com/7th-cpc-transport-allowance-and-da-granted-for-indian-railway-medical-service-irms-officers-drawing-grade-pay-of-rs-10000-under-dacp-scheme/" target="_blank" rel="noreferrer noopener sponsored nofollow">7th CPC Transport Allowance and DA granted for Indian Railway Medical Service (IRMS) officers drawing Grade Pay of Rs.10,000/- under DACP Scheme</a></strong></p>



<p>3) Railway Hospitals performed pre-placement medical examination for 38,000 candidates and carried out periodical medical examination for 1.14 lakh employees. They also examined 12.51 lakh food and water samples.</p>



<p>4) Indian Railways total medical services expenses for the year 2018-19 including cost of medical staff is Rs. 6888.34 crore. That makes average medical expense per beneficiary as Rs.10,500.</p>



<p>5) No medical insurance company in India handles 66 lakh beneficiaries. Medical Insurance doesn’t cover OPD. Since Railway is catering 2.01 crore OPD patients in a year, it may not be possible to include huge OPD beneficiaries in private managed medical insurance.</p>



<p>6) In existing Railway medical system all medical services like OPD, diagnosing, medicines, Surgeries, in-patient care, major treatment/surgeries carried out in referral private/ CGHS hospitals and follow-up checking are part of employees’ welfare scheme offered to employees and their dependents. It is a part of service condition. For retired employees and their dependents who opted for RELHS on onetime payment, same medical services are extended.</p>



<p>7) In the Railway Medical system, no cost ceiling is fixed for entitlement of any required treatment. On the other hand, medical insurance cover is basically extended to in-patient treatments and for surgeries with a ceiling based on the premium paid.</p>



<p>8) For example, United India Insurance Company Limited offers Rs.10 lakh sum insured at premium rate per member Rs.12,386 in age group 41-45. If all 66 lakh Railway beneficiaries are covered under above premium Rs.8370.78 crore will be paid as total premium. Whereas total medical expense of Indian Railways for the year 2018-19 is Rs. 6888.34 crore which is for diverse services given in the Annexure.</p>



<p>9) In the same time there is a need for addition of more private empanelled hospitals in Indian Railways Medical Services with seamless cashless treatment, since large number of serving and retired health care beneficiaries live in the newly developed suburbs of various cities throughout India. It is pertinent to note that in case of emergency, beneficiaries first require to approach Railway Hospital, then they are referred to private hospitals and in many cases precious golden hours are lost in mere formalities endangering the life.</p>



<p>10) Beneficiaries living in faraway places from Railway Hospitals also find it difficult to get their regular treatments and minor treatments occasionally, since they are required to travel long distance to reach Railway Hospitals. These beneficiaries will be benefited if cashless treatment is extended to them in the hospitals available near their living place.</p>



<p>11) Hence it is requested that,</p>



<p>a. Existing Indian Railways Medical Service facilities may please be continued.</p>



<p>b. The proposal for Medical Insurance Scheme for Railway employees / Pensioners and their depended may please be made as additionally facility to the beneficiaries to get their regular treatments including OPD &amp; IPD in empaneled private hospitals, premium thereof may please be borne by Railways.</p>

<p>Indian Railways has geared up its entire machinery in a big way to take comprehensive measures on prevention of Corona COVID 19 virus infection.  Minister of <a href="https://centralgovernmentnews.com/category/railways/" target="_blank" rel="noreferrer noopener" aria-label="Railways (opens in a new tab)">Railways</a> and Commerce &amp; Industry, Shri Piyush Goyal has directed the Railway Board to ensure that adequate prevention measures are put in place across Indian Railway system. The Chairman Railway Board, Shri Vinod Kumar Yadav, has taken meeting with all General Managers through Video Conference on 5th and 6th March 2020 to review the preparedness and convey priorities and directions.</p>



<p>Heads of Railway Zones, Divisions and Units have been taking meetings in their respective areas to firm up necessary arrangements. The situation is being constantly monitored and coordinated at all levels in the Indian Railway system i.e. at Railway Board level, zone level and at Division level. Control rooms have been set up all over the railway system and the staff has been sensitized and educated about the subject. The collaboration and coordination with the concerned health authorities is also being ensured.</p>



<p><strong>Following action has been taken for prevention of Coronavirus (COVID-19) infection by Indian Railways.</strong></p>

<ul class="wp-block-list"><li>Information, Education, &amp; Communication (IEC) material (posters and pamphlets) regarding corona virus (COVID-19) are being prominently displayed for awareness of general public in local language at railway stations and in trains and are also being distributed to patients visiting hospitals and in Railway colonies. Audio and video clips are being played at railway stations to spread awareness. Public announcements are being made at stations.</li><li> In Railway hospitals, Persons with fever are being separated from other patients. Separate counter / ward with specific signage for fever cases have been established. Medical staff has been deployed at such areas having proper protective gears.</li><li> Railway hospitals have been provided with 1100 isolation beds for fever related cases for treating suspected Corona virus cases with availability of necessary protective gears. Further, Railways has identified 12483 beds for quarantine at different locations across the country.</li><li> Medical staff have been advised that if any patient of Corona virus disease is suspected or reported in any of the Railway Hospital / Health Unit in Zone / PU, the same may be informed to Railway Board and local health authorities immediately.</li><li> All medical in-charges of Zones /PUs have been advised to be in constant touch with the respective State Authorities to obtain the guidelines / updates issued on the subjects and take necessary detection, prevention &amp; curative measures suggested by such State Authorities.</li><li> Control rooms and Telephone helplines have been set up at all Railway hospitals.</li></ul>

<p>Kind attention is invited to the correspondence cited above on NFIR&#8217;s PNM item No. 12/2016 pending with the Railway Board. Though the subject was discussed with the Railway Board (ED/PC-I) in the special meeting held on 04/06/2019 and Official Side conveyed that the matter was under consideration, unfortunately the issue has not yet been settled.</p>

<p>Federation further desires to convey to the Railway Board that pursuant to Railway Board&#8217;s instructions revising the designations of various erstwhile Group &#8216;D&#8217; Staff (RBE No. 201/2018 dated 27/12/2018), the following categories of staff working in the Railway Hospitals have been excluded from the payment of HPCA/PCA.</p>



<p><strong>1) Hamals, GP 1800 and 2) Peon Assistants, GP 1800 now revised as Hospital Attendant.</strong></p>



<p>In this connection, NFIR desires to state that the staff of above categories working in the Railway Hospitals also undertake various jobs and come into contact with the patients, therefore they should also be covered for payment of Hospital Patient Care Allowance / Patient Care Allowance.</p>



<p>NFIR, therefore, request the Railway Board again to review and issue instructions to the Zonal Railways etc., soon covering all the remaining categories of staff working in the Railway Hospitals for the purpose of payment of Hospital Patient Care Allowance / Patient Care Allowance as has been allowed to the staff of categories working in the Central Government Hospitals.</p>

<p>The Railway Board vide reply sent to the Federation under letter dated 27/02/2018 (NFIR’s PNM Agenda Item No. 1-B/2012) on payment of Special Allowance to Nursing Personnel working in Operation Theatre/Incentive Care Units etc., has admitted that the issue is still under consideration in consultation with MoH &amp; FW, the nodal Ministry, whose reply is still awaited despite several reminders and the Ministry of Railways cannot take decision for revision of rates of the Special Allowance from Rs. 120 p.m. to Rs. 360 pm. unilaterally and that as and when such a communication is received from MoH &amp; FW, the rates will be revised. Similar position has also been conveyed to the Federation during NFIR’s PNM meeting held with the Railway Board on 10/11-05-2018 through Action Taken Statement as the Agenda Item could not be discussed due to paucity of time. Federation feels sad that though a period of more than ten months passed, there has been no progress in the matter, while the subject is under discussion since over six years.</p>



<p>In this connection, Federation places following points for consideration in order to take independent decision on the issue:-</p>



<p><strong>The 6th CPC vide Para No. 4.2.81 relating to Miscellaneous Allowances made following recommendation :-</strong></p>



<p>“Similarly, rates of Allowances specific to different Ministries/Departments/ Organizations not covered in this report will also be doubled. The rates of these Allowances will be increased by 25% every time the Dearness Allowance payable on revised pay scales goes up by 50%”.</p>



<p>The above recommendation of the 6th CPC was accepted by the Government as notified in the Gazette of India: Extraordinary-29/08/2009.</p>



<p>Consequently, the operation Theatre Allowance (Special Allowance) to Nursing Personnel working in the Operation Theatres/ICUs etc., should have been doubled from Rs.120 p.m. to Rs.240 p.m. w.e.f. 01-09-2008. This amount should have been revised to Rs.300 p.m. w.e.f 01-01-2011 due to DA hike upto 50%. Similarly, the Special Allowance should have again been revised to Rs.360 p.m. w.e.f. 01-01-2014 as the DA touched 100% of pay.</p>



<p>It is noteworthy to appreciate that the Railway Board have issued orders independently, revising the rates of Operation Theatre Allowance and other Specialized Allowances to the Nursing Personnel working in the Railway Hospitals vide letter dated 27-12-2017, consequent upon acceptance of the recommendations of 7th CPC.</p>



<p><strong><em>Note</em></strong>: Railway Board may also take into account that many Nursing personnel have since retired after 01-09-2008, therefore they should also be covered for payment of the Special Allowance.</p>



<p>Federation also brings to the notice of Railway Board that the first O.M.vide No.E(P&amp;A)I-2009/SP-1/Genl/1 dated 09-07-2010 was sent by the Railway Ministry to the Ministry of Health &amp; Family Welfare i.e more than nine years ago which shows that abnormal delay had already been caused in the matter while the Allowance as per the 7th CPC recommendations have already been implemented.</p>



<p>NFIR again requests the Railway Board to consider the above points and accord approval, revising the Special Allowance from Rs.120 p.m to Rs.240 p.m., Rs.300 p.m and Rs.360 p.m. to the Nursing Personnel with effect from the dates mentioned above. A copy of the order may be endorsed to the Federation.</p>

<p>Kind attention is invited to the comments of Railway Board vide Annexure OA of Board&#8217;s reply dated 14/03/2019, not agreeing for providing cadre structure for ECG Technicians with apex Grade Pay 4200/pay Level-6, on the ground that no specific recommendations have been made by the Pay<br /> commission in favour of ECG Technicians of Railways, the parity between various categories of para-medical staff working in Railway Hospitals/Health Units cannot be made with para-medical or para-medical staff of specified Central Government Hospitals due to functional requirements. These views of the Railway Ministry have been disputed by the Federation earlier, due to being incorrect &amp; unjust and urged upon the Railway Board to take judicious decision to render justice to the ECG Technicians working in Railway Hospitals/Health Units. In this context, Federation&#8217;s letter dated 26/09/2016 and 30th July, 2018 may kindly be referred to. </p>



<p>Railway Board may also appreciate that the Railway Hospitals mainly Zonal and Divisional Hospitals have been functioning on at Zones and the Divisions. These Railway Hospitals have been providing Cardiac and other important treatments to ailing Railway employees and their family members as indoor patients located in all big cities in the Zones/Divisions. Besides this, job contents of the ECG Technicians and their role/service conditions in these Railway Hospitals is in no way less than the Medical Institutions of National Eminence. It is also relevant to appreciate that most of these Railway  Hospitals are having 100 plus bed strength giving all types of treatment to the patients, therefore, the ECG Technicians working in Railway Hospitals cannot be rated inferior to those ECG Technicians working in other National Medical Institutions but rather deserve to be considered atleast on par if not more so far as their pay structure is concerned. In this connection, Railway Board may kindly consider Federation&#8217;s proposal dated 26/09/2016 &amp; 30/07/2018 (copies enclosed) and take the issue to the nodal Ministry i.e. Ministry of Health and Family welfare with justification for allotment of Pay Level-6 at apex level to the ECG Technician category already placed in the NFIR PNM Agenda Item No. 60/2012.</p>



<p>Another fact which cannot be ignored is that at present the Central Hospitals working on each Zonal Railway have been functioning as Educational Institutions also as the Graduate/Post Graduate Doctors are undergoing DNB, internship etc., and other types of courses before they being awarded Degree. These Railway Hospitals cannot be under-rated in so far as their work culture and responsibilities shouldered by staff are concerned. While several cadres in Railways where entry Grade Pay is 1800/- and 1900/- with direct recruitment qualification being lower than &#8220;Graduation&#8221; are provided cadre structure upto GP 4200 and in some cases GP 4600, in the case of ECG Technicians where entry qualification is &#8220;Graduation in Science plus Diploma/experience&#8221; apex Grade Pay 4200/- has not been provided in the existing cadre structure. This is unique case of injustice meted out to the category despite V CPC had given general recommendation for allotment of &#8220;qualification based pay scale&#8221;. The case of ECG Technicians therefore stands on merit for providing improved cadre structure with atleast GP 4200/ Level-6 at apex level of the cadre.</p>



<p>NFIR, therefore, once again requests the Railway Board to kindly review and make out a proposal for obtaining the approval of Ministry of Health &amp; Family Welfare and MoF as suggested above.</p>

<p>Kind attention of Railway Board is invited to NFIR&#8217;s PNM agenda item No. 1212016 and the correspondence cited under reference.</p>
<p>In this connection, Railway Board vide letter dated 1310912018 conveyed to the Federation that a Committee had been constituted to find out if any other category of staff working in Railway Hospitals Hlealth Units is eligible for HPCA/PCA and the recommendations of the Committee are being processed for further necessary action. Consequently, NFIR vide its letter dated 1910912018 requested the Railway Board to make available a copy of the recommendations of the Committee to facilitate the Federation to go through the same and make appropriate suggestions. A period of more than three months passed, copy of the Committee&#8217;s report has not been made available to the Federation.</p>
<p>NFIR, therefore, once again requests the Railway Board to please provide a copy of the recommendations of the Committee early for Federation&#8217;s perusal for discussing further, with the Board.</p>

<p>Sub: <strong>Issuance of Special Passes on Medical grounds for follow-up medical treatment in outstation Hospitals.</strong></p>
<p>It has been brought to the notice of Board that difficulties are being faced by serving/retired railway employees &amp; their family members due to the present practice or issuing of Special Passes on medical grounds for follow-up medical treatment. Such passes are being issued just a few days before the due date of visit to specialized hospitals located in outstations. Due to this practice, the beneficiaries of such Special Passes are unable to secure confirmed advance reservation.</p>
<p>2. The matter has been examined in consultation with Health Dte. of the Ministry of Railways. Keeping in view the fact that the beneficiaries of such passes are normally those who are suffering from serious ailments like cancer, renal failure, heart ailments, etc. and in order to facilitate securing of advance reservation for them, it has been decided to stipulate the following procedure for issuing of Special Passes [under Schedule VII of Railway Servants (Pass) Rules, 1986 (Second Edition 1993) for follow-up medical treatment:-</p>
<blockquote><p>(i) The Competent Medical Officer in the Railway Hospital will issue necessary prescription on the basis of follow-up visit advice recorded by the outstation Hospital, as and when approached by the beneficiary, without waiting for the due date for follow-up visit to come nearer. In the said prescription, the period of travel validity (which is to be endorsed on the pass) must be indicated clearly, which shall be one week i.e. 7 days before and 21 days after the clue date of visit. For example, If a beneficiary has to reach a Hospital in Mumbai for follow-up treatment on 8th December 2018, the Medical Officer concerned should write in the prescription: &#8220;<strong>Special Pass valid for travel from 1st Dec, 2018 to 29th Dcc, 2018 may be issued</strong>.&#8221;</p>
<p>(ii) The PASS Issuing Authorities, on the basis of Aforementioned prescription and recommendation will issue Special Pass upto 5 months in advance endorsing thereon the travel validity period as &#8220;Valid for travel from __/__/_____ to __/__/_____&#8221; (i.e. the dates as recommended in the prescription).</p></blockquote>
<p>3. Moreover, representations are also being received in Board&#8217;s office. from the beneficiaries of Special Passes issued on medical grounds complaining of delays in issue of such passes. In this context, the Railways are directed to sensitize the concerned pass issuing officials to issue the Special Passes on medical grounds on the very same day of receipt-of the application in case of serious ailments and at the most, next day in all other cases.</p>

<p>Kind attention of the Railway Board is invited to the minutes of NFIR&#8217;s PNM meeting held with the Railway Board on l0th/1lth May,2018, pertaining to Agenda Item No. 1812018, wherein the Official Side conveyed that after finalization of revision of entry pay of Dressers (proposal presently being pursued with MoF), this demand can be looked into. Though five months passed, unfortunately no progress has been made relating to grant of entry pay of GP 2000/Level-3 in respect of Dressers/OT Assistants working in the Rai lway Hospitals/Health Units.</p>
<p>In this connection, Federation desires to bring to the notice of Railway Board that the Ministry of Health and Family Welfare, (the nodal Ministry), has since approved restructuring of the cadre of Operation Theatre in one of the Hospitals vide letter No. V-17020118912018-INI-II dated 7th September, 2018 as follows:-</p>
<p><strong>(i) OT Technician: Entry Level Scale of Pay (PB-2+GP 4200/Pay Matrix Level-6).</strong><br />
<strong>(ii) Sr. Technician OT (on promotion from GP 4200) to be placed in PB-2+GP 4600.</strong><br />
<strong>(iii) Sr. Technician OT is eligible for promotion as Technical Officer (OT) in PB-2+GP 5400. (iv) The Technical Officer (OT) GP 5400 is eligible for promotion as Sr. Technical Officer (OT) in PB-3+GP 6600.</strong></p>
<p>Federation suggests that the Railway Board may consult the Ministry of Health &amp; Family Welfare for formulating structure for allotment of GP 4200 in favour of Dressers/OT Assistants.<br />
NFIR, therefore, requests the Railway Board to take necessary action at the level of Ministry of<br />
Health &amp; Family Welfare.</p>

<p>Sub: <strong>Debit and Credit note for treatment of of patient (Railway Medical Beneficiary) to other Railway Hospital with or without referral &#8211; Simplification of Process.</strong></p>
<p>It has been reported that the treatment of patient (Railway Medical Beneficiary) has been delayed because of the demand of debit note by Railway hospital where patient has been referred or reported, from the Railway/Unit/Hospital to which the patient belongs. In order to facilitate easy treatment of Railway Medical Beneficiaries, Board (MS, FC &amp; CRB) have approved the following:</p>
<p>1. Referral Case: A Railway Hospital referring the patient (Railway Medical Beneficiaries) to other Zonal Hospital will include a note in its referral from/letter that it will accept debit raised on the treatment of the patient at Railway Hospital and/or further onward referral of the patient to empanelled hospitals and/or to non empanelled hospitals by making advance payment. All the necessary sanctions, if required, will be taken at the referred Railway itself.</p>
<p>2. Reported without Referral: In case patient (Railway Medical Beneficiary) has reported to the hospital of other Railway/unit without referral &#8211; treatment will be provided by the hospital – in house and/or referral to empanelled hospital and/or empanelled hospital as advance payment case, as the need be after ascertaining the valid identity of patient (Railway Medical Beneficiary) thru Medical ID Card/ ID Card/ Pass/ PTO etc. All the necessary sanctions, if required, will be taken at reported Railway itself.</p>
<p>Meanwhile the reported Railway/Unit/Hospital will inform the concerned Railway/Hospital/Unit to which the patient originally belongs.</p>
<p>3. The patient (Railway Medical beneficiary) will be given the treatment as per the medical condition by the referred or reported Hospital and after treatment is over, then the debit will be raised on the concerned Zonal Railway/Unit/Hospital to which the patient originally belongs.</p>
<p>This issues with the concurrence of Associate Finance of Transformation Cell of Railway Board.</p>
<p>Kindly acknowledge the receipt and ensure compliance.</p>
