<p>Sub: <strong>Entry / Updation of Family details in Service Book</strong></p>



<p>It has been observed that some of the sub offices are casually forwarding the family details of the employees for updation in Service Books which is resulting in undue correspondence. Hence, the following instructions are hereby issued which are required to be followed while forwarding the family details for updation in the service records:</p>



<ol class="wp-block-list"><li>The family details are being received on different proforma from different offices. Therefore a common format of &#8216;Details of Family&#8217; has already been uploaded on the website of <strong><a href="https://centralgovernmentnews.com/tag/PCDA/" target="_blank" rel="noreferrer noopener">PCDA </a></strong>(WC) Chandigarh and details should be forwarded in the prescribed format only.</li><li>Proof of DOB of every dependent should be enclosed with the request for entry / updation of family detail.</li><li>In case of dependent Parents, the income certificates of the parents/ affidavit sworn in before 1st Class Magistrate duly mentioning that the joint income of parents from all sources is less than Rs. 9000/- plus applicable DA.</li><li>If any of the dependent children is handicapped, the Certificate issued by concerned Medical Authorities should be enclosed.</li><li>In case spouse is employed in State/ Centre Govt. a joint declaration regarding claim of Tuition fee/ Medical reimbursement/ LTC should be enclosed (Format available on www.pcdawc.gov.in )</li><li>In case the family details-are present in the service records and further updation is required due to any addition/deletion in family the same should be supported by requisite documents (e.g Birth Certificate/ Divorce Decree etc.) and names of all present family members should be reflected in the prescribed format for the purpose.</li><li>The Family details should be countersigned by the Officer-In-Charge of the sub office.</li><li>It should also be ensured that the employee has tick marked / cross marked all five declarations provided in the format.</li></ol>

<p>Subject: <strong>Relaxation of procedures to be followed in considering requests for medical reimbursement &#8211; Clarification</strong></p>
<p>The undersigned is directed to state that the condition mentioned in Para 3(2)(g) of this Ministry&#8217;s OM No. 4-18/2005-C&amp;P [Vol.1I-Pt (I)], dated 20.02.2009 is modified to the extant as under:</p>
<p>&#8220;The treatment was obtained in private non-empanelled hospital under emergency, while on official tour to another city.&#8221;</p>
<p>2. Further, it is also clarified that the cases, which were processed and decided earlier on the basis of OM dated 20.02.2009, should not be reopened.</p>
<p>3. This OM will be effective from the date of issue.</p>
<p>4. This issues with the approval of the Competent Authority.</p>

<div>During the audit of Medical Claims pertaining to your office some shortcomings have been noticed to your office which are appended below.</div>
<div></div>
<div>1. The medical claims are submitted along with the Hospital Bills without PAN,TIN,TAN printed on the Bills.</div>
<div></div>
<div>2, The claims/treatments of Emergency nature are submitted without any assessment from the Addition A Director of the concerned CGHS cities.</div>
<div></div>
<div>3. Most of the claims of more than RS,2 lacs are forwarded to this office without necessary permission from the HOD, New Delhi/Ministry of Hearth and Welfare, New Delhi.</div>
<div></div>
<div>4.The Medical claims are submitted in a casual nature without enclosing the BHT sheet duly verified by the treating doctor</div>
<div></div>
<div>5.. CGHS Hospitals are not giving 10% discount on every cash payment.</div>
<div></div>
<div>To avoid the return of the medical claims repeatedly and inconvenience faced by the individuals the o I/C of the units/formations you are advised to rectify the shortcomings before forwarding the claims to this office. And it has been decided by the competent authority that the units/formations may liaise with hospitals of the concerned cities for bills/documents bearing printed PAN,TAN,TIN(wherever applicable). It has also been decided that no medical claims will be entertained w.e.f 01/07/2014 if printed PAN,TIN TAN(where applicable) are not embossed on the bilis.</div>

										<content:encoded><![CDATA[<div dir="ltr"><strong>‘Reimburse government staff for private treatment’</strong></p>
<p>A central government servant is entitled for reimbursement even if he takes treatment in a private hospital under emergent situation, the TN Bench of the Central Administrative Tribunal has held.</p>
<p>M Mohamed Salia, Deputy Chief Engineer, Southern Railway, while returning home, suffered a heart attack on November 20, 2008. Due to the urgency of the matter, his wife admitted him in the nearest private hospital Frontier Lifeline, as the Railway Hospital was 10 km away from her residence. After a by-pass surgery and necessary treatment, he was discharged on December 12, 2008. He paid Rs.3.10 lakh towards hospital bills.</p></div>

<div>When he applied for reimbursement of Rs.2 lakh to which he was entitled, the railway authorities rejected his claim on the ground that treatment in a non-recognised private hospital without referral by the railway authorised medical officer was not admissible. Hence, the present application.</div>

<div>Rejecting the contentions, CAT judicial member G Santhappa said that in this case, the applicant had produced the emergency certificate and that had not been considered by the railways. The Personnel Branches Circular (PBC) dated May 4, 1994 listed under what circumstances reimbursement of medical expenses could be made. It included that if a patient falls ill at a place where there was no government or railway hospital and that if transporting the patient to the nearest government hospital would result in loss of life, the servant could be admitted in a private hospital. The rejection was against the law laid down by the SC, the tribunal said, set aside the order and directed the railways to sanction the amount in a month.</div>
