Sub: PROPOSAL OF MEDICAL INSURANCE SCHEME FOR RAILWAY EMPLOYEES.

1) Ministry of Railways constituted a committee to examine and tender their recommendations to bring all Railway Employees under the Medical Insurance Scheme for obtaining treatment in private hospitals. IRTSA submits the following for the kind consideration of Railway Board,

2) Indian Railways Medical Service (IRMS) is spread across the length and breadth of the country. IRMS is having 125 hospitals and 586 dispensaries, manned by 33,300 medical staff catering to approximately 66 lakh beneficiaries including 12.27 lakh serving employees. In the year 2018-19 Railway Hospitals treated 2.01 crore OPD patients, 5.08 lakh in-patients and performed 1.02 lakh major and specialty surgeries.

3) Railway Hospitals performed pre-placement medical examination for 38,000 candidates and carried out periodical medical examination for 1.14 lakh employees. They also examined 12.51 lakh food and water samples.

4) Indian Railways total medical services expenses for the year 2018-19 including cost of medical staff is Rs. 6888.34 crore. That makes average medical expense per beneficiary as Rs.10,500.

5) No medical insurance company in India handles 66 lakh beneficiaries. Medical Insurance doesn’t cover OPD. Since Railway is catering 2.01 crore OPD patients in a year, it may not be possible to include huge OPD beneficiaries in private managed medical insurance.

6) In existing Railway medical system all medical services like OPD, diagnosing, medicines, Surgeries, in-patient care, major treatment/surgeries carried out in referral private/ CGHS hospitals and follow-up checking are part of employees’ welfare scheme offered to employees and their dependents. It is a part of service condition. For retired employees and their dependents who opted for RELHS on onetime payment, same medical services are extended.

7) In the Railway Medical system, no cost ceiling is fixed for entitlement of any required treatment. On the other hand, medical insurance cover is basically extended to in-patient treatments and for surgeries with a ceiling based on the premium paid.

8) For example, United India Insurance Company Limited offers Rs.10 lakh sum insured at premium rate per member Rs.12,386 in age group 41-45. If all 66 lakh Railway beneficiaries are covered under above premium Rs.8370.78 crore will be paid as total premium. Whereas total medical expense of Indian Railways for the year 2018-19 is Rs. 6888.34 crore which is for diverse services given in the Annexure.

9) In the same time there is a need for addition of more private empanelled hospitals in Indian Railways Medical Services with seamless cashless treatment, since large number of serving and retired health care beneficiaries live in the newly developed suburbs of various cities throughout India. It is pertinent to note that in case of emergency, beneficiaries first require to approach Railway Hospital, then they are referred to private hospitals and in many cases precious golden hours are lost in mere formalities endangering the life.

10) Beneficiaries living in faraway places from Railway Hospitals also find it difficult to get their regular treatments and minor treatments occasionally, since they are required to travel long distance to reach Railway Hospitals. These beneficiaries will be benefited if cashless treatment is extended to them in the hospitals available near their living place.

11) Hence it is requested that,

a. Existing Indian Railways Medical Service facilities may please be continued.

b. The proposal for Medical Insurance Scheme for Railway employees / Pensioners and their depended may please be made as additionally facility to the beneficiaries to get their regular treatments including OPD & IPD in empaneled private hospitals, premium thereof may please be borne by Railways.

Attention is invited to Board’s letter of even number dated 25-08-2015 whereby Railways were advised to keep in abeyance the payment of Transport Allowance @ Rs.7000/-p.m. + DA thereon to IRMS officers drawing Grade Pay of Rs.10,000/- under DACP Scheme. Subsequently, as per the interim order dated 27-10-2015 of Hon’ble CAT/New Delhi in O.A No.3528/2015, Railways were advised vide this office letter of even number dated 06-11-2015 that the payment of Transport Allowance to IRMS officers @Rs.7000/-p.m. + DA thereon, be continued till further orders.

2. The Hon’ble CAT/ New Delhi vide order dated 28-05-2019 has now disposed of the said O.A No. 3528/2015 has observed as under :-

“The OA is, therefore, disposed of directing the respondents to take a final decision regarding extension or otherwise of the Transport Allowance at the rate of Rs: 7000/- per month plus DA, to the members of the applicant Association, within a period of three months from the receipt of this order. However, the amount already paid towards the said allowance shall not be recovered, and the respondents shall not be under an obligation to pay the said allowance until a final decision is taken. There shall be no orders as to costs.”

3. The matter has been accordingly considered by the competent authority and in view of fact that IRMS officers drawing Grade Pay of Rs.10,000/- (now pay level -14) under DACPS except those who are holding pinpointed administrative SAG posts mentioned in Board’s letter No.2009/H/16/1DACP dated 26-02-2009, are not entitled for use of official car in terms of Board’s letter No.E(G)95AL4-9, dated 06-11-1995, it has been decided that such officer shall not be eligible to opt for higher rate of Transport Allowance @Rs.7000/- p.m.+DA thereon (which now stands revised to Rs.15750/- p.m.+DA thereon based on the recommendations of the 7th CPC). These officers are entitled for Transport Allowance @Rs.7200/3600 + D.A thereon as per row- 2 of Board’s letter of even number dated 3-8-2017.

Sub: Extension of retirement age of Doctors- Travel Entitlements reg.

In terms of Board’s letter No. E(P&A)1-2016/RT~16, dated 20/09/2018 & 17/10/2018, doctors belonging to IRMS and Dental Doctors-under the Ministry of Railways has been provided opportunity to serve the Government upto 65 years under certain conditions on their exercising the option of posting to a clinical post.

In this connection, it has been decided that those doctors belonging to IRMS and Dental Doctors under the Ministry of Railways who transferred to clinical duties on attaining the age of 62 years will carry their travel entitlements in the same manner as it was prior to such extension.

Sub: Option to be submitted by Doctors belonging to IRMS and Dental Doctors under the Ministry of Railways to serve the Government upto 65 years – Reg.

Ref: 1. Ministry of Personnel, Public Grievances and Pensions (DoP&T)’s Notification No. GSR.767(E) dated 11.08.18 (Annexure A).

2. MoH&FW’s OM NO. A.12034/4/2018-CHS.V dated 13.8.18 (Annexure B).

In supersession of orders contained in Board’s Letters of even number dated 31.05.16 (RBE No. 57/2016) and 26.12.2017 (RBE No. 205/2017) and in accordance with the orders contained in the DoP&T’s Notification No. GSR.767(E) dated 11.08.18 amending Clause (bb) under FR 56 of the Fundamental Rules, 1922, the provisions in respect of age of superannuation of doctors of the Indian Railways Medical Service and Dental Doctors under the Ministry of Railways are modified as stated in the Notification.

2. The age of superannuation of Doctors belonging to IRMS and Dental Doctors under the Ministry of Railways shall be sixty-two (62) years unless they exercise the option of posting to a Clinical post depending upon their expertise and experience, as decided by the Competent Authority in the Ministry of Railways from time to time, in case they desire to continue in their service upto the age of sixty-five (65) years.

3. The serving doctors belonging to IRMS and Dental Doctors under the Ministry of Railways who have already attained the age of 62 years or will be attaining the age of 62 years within 6 months from the date of issue of these orders may exercise their option as per the enclosed format within a period of 30 days from the date of issue of these orders. All other doctors would, in future, be required to submit their option not less than 06 months before their attaining the age of 62 years. The Railway doctors should furnish their option through the Personnel Branches of their respective Railway administrations.

4. The serving doctors who fail to exercise the option within the period specified above shall stand superannuated from service on attaining the age of 62 years or on expiry of the period of 30 days from the date of issue of these orders, whichever is later.

5. Posting orders involving inter-Railway transfers and for those working in Boards office will be issued by the Railway Board. Orders of CMSs and CMDs in Zonal Railways who cross 62 years of age and opt to serve in a Clinical post and who are posted on the same Railway itself would be issued by the Zonal Railway administration themselves.

6. Copies of options exercised by the Railway doctors should be forwarded to the Board’s office (Secretary’s Confdl. Cell) for record and necessary action.

7. A list of administrative posts identified by the Ministry of Railways and designations of IRMS officers after 62 years is annexed (Annexure C).

8. These instructions are available online at www.indianrailways.gov.in.

9. These instructions may be brought to the notice of all doctors belonging to IRMS and Dental Doctors under the Ministry of Railways.

10. This issues with the concurrence of Finance Directorate of the Ministry of Railways.
