										<content:encoded><![CDATA[<p><b>Poor Functioning of CGHS</b></p>
<p>Some grievances have been received from CGHS beneficiaries through grievance portals and other channels regarding functioning of CGHS. These are mainly regarding non-issue of medicines, plastic cards, shortage of doctors/ specialists and long queue of patients in Wellness Centres.</p>
<p><b>Following action has been taken by the government in this regard:</b></p>
<blockquote class="tr_bq"><p>(i) Appointment of retired doctors on contract basis as a stop gap arrangement to fill the vacant posts.</p>
<p>(ii) Medicines, which are not available at CGHS Wellness Centres are procured through Authorized Local Chemists.</p>
<p>(iii) A special drive was undertaken to clear the pendency of issue of CGHS Plastic cards</p>
<p>(iv) Option for self-printing of CGHS cards for existing CGHS beneficiaries.</p>
<p>(v) Introduction of on-line registration for consultation at selected Wellness Centres in Bengaluru and in one zone in Delhi.</p>
<p>(vi) Initiation of tele-consultation on trial basis in Delhi from two CGHS Wellness Centres with specialists of Dr. R.M.L Hospital.</p></blockquote>
<p>Regular internal audits and also external audits/studies on the functioning of CGHS have been carried out.</p>
<p><b>Outcome of some of the important studies are as under:</b></p>
<p>(i) Staff Inspection Unit (SIU) was carried out by Ministry of finance for Staff Pattern Norms based on patient attendance.  Recommendations of the SIU have been implemented.</p>
<p>(ii) Based on the study carried out by Kaul Committee, computerization of all the CGHS Wellness Centres has been done</p>
<p>(iii) On the basis of recommendations of the CAG Audit on procurement of medicines, Medical Stores Organization (MSO) has initiated e-tender for procurement of Anti Cancer and other Lifesaving medicines. It has also been decided to procure only L1 Rate medicines under formulary for Branded Medicines.</p>
<p>(iv) Committee of Secretaries (COS) has also been monitoring the functioning of CGHS since 2008 and various steps have been implemented to make CGHS beneficiary friendly. As per the recommendations of COS, UTI-ITSL has been appointed as Bill Clearing Agency for settlement of credit bills of empanelled hospitals and diagnostic centres pertaining to the treatment of CGHS pensioner beneficiaries.</p>
<p>(v)  The functioning of CGHS is also monitored by the Hon’ble Prime Minister and as per the directions under ‘PRAGATI’, the process of linking of Aadhar Number with CGHS beneficiary Numbers has been started.</p>
<p>A special drive was undertaken to clear the pendency of issue of CGHS Plastic cards. Provision has been made for self-printing of CGHS cards by existing CGHS beneficiaries.</p>
<p>(vi) DAR&amp;PG conducted a study through Quality Council of India (QCI)   to identify   areas of Grievances in Different Ministries and organizations including CGHS and suggested possible solutions.</p>
<p><b>Based on the inputs so received:</b></p>
<ul data-blogger-escaped-style="text-align: left;">
<li>Financial Powers of Additional Directors of CGHS have been enhanced for early settlement of medical claims of Pensioners.</li>
<li>Introduction of on-line registration for consultation at selected Wellness Centres in Bengaluru and in one zone in Delhi.</li>
<li>Aadhar-based Bio-metric attendance for the staff at CGHS Wellness Centres has been implemented in Delhi and NCR.</li>
</ul>
<p><b>The following steps have been taken for improvement in the functioning of CGHS in the country:</b></p>
<p>1. Regular Inspection of Wellness Centres is carried out.</p>
<p>2. Monthly advisory committee meetings are held for each Wellness Centres with CMO   (Incharge)</p>
<p>3. Regular monitoring of empanelled hospitals by a team constituted by Additional Directors, CGHS of cities for this purpose.</p>
<p>4. Regular meeting of Additional Directors, CGHS with pensioner associations.</p>
<p><b>SMS alert facility to CGHS beneficiaries.</b></p>
<p>The Minister of State (Health and Family Welfare), Sh Faggan Singh Kulaste stated this in a written reply in the Lok Sabha here today.</p>

<p>Sub: <strong>Renewal of CGHS Plastic Cards – reg.</strong></p>
<p>The undersigned is directed to refer to this Ministry’s O.M No.S.11012/3/2011-CGHS (P) dated 29.12.2011 laying down the guidelines for issue of individual plastic cards to CGHS beneficiaries. CGHS Plastic Cards were introduced in September, 2008 in Delhi NCR and the cards were initially issued with a validity period of 5 years. The CGHS Plastic Cards completing their validity period are due for renewal and accordingly fresh cards with renewed validity period are being issued by the Office of Additional Director, CGHS of the city concerned. With a view to further streamline the process of renewal of CGHS Plastic Cards, it has been decided to issue the following guidelines supplementing the existing instructions on issue of CGHS Plastic Cards:</p>
<p style="text-align: center;"><span style="text-decoration: underline;"><strong>Serving beneficiary</strong></span></p>
<p>a) Application for renewal of CGHS Plastic Cards in the prescribed proforma (Form AA) alongwith requisite documents (current photographs, copy of pay slip and address proof of residence, if changed), should be submitted through their Administrative Office to the Office of Additional Director, CGHS of the respective CGHS city following the same procedure as prescribed for issue of fresh CGHS card.</p>
<p>b) Fresh CGHS Plastic Cards with same beneficiary ID nos. shall be issued with a validity period of 5 years.</p>
<p>c) Applications for renewal of old plastic cards can be made 3 months in advance prior to its expiry.</p>
<p><strong></strong></p>
<p style="text-align: center;"><span style="text-decoration: underline;"><strong>Pensioner beneficiary</strong></span></p>
<p>a) Application for renewal of Pensioners’ CGHS Plastic Cards in the prescribed proforma (Form BB) alongwith requisite documents (current photographs, PPO or LPC, address proof, if changed) should be submitted to the parent CGHS Wefiness Centre where his /her card is registered. He / she can also submit the application to the Office of Additional Director, CGHS of the respective CGHS city for renewal of CGHS cards.</p>
<p>b) Fresh CGHS Plastic Cards with same beneficiary ID nos. shall be issued with validity for lifetime or up to the date for which the contribution has been made by the beneficiary.</p>
<p>c) Applications for renewal of old plastic cards can be made 3 months in advance prior to its expiry. This issues with the approval of Additional Secretary and Director General, CGHS.</p>

While answering in the Parliament to a question regarding above mentioned the topic, the Minister of Health and Family Welfare Shri Ghulam Nabi Azad said that &#8216;Proof of Residence is required for issuing CGHS cards. However, Affidavits are also accepted in absence of commonly accepted proof of residence.</p>
<p>He also said that CGHS cards in respect of serving employees and their dependent family members are issued on the recommendation of their Administrative Department / Office. It is incumbent upon the Administrative Department / Office to ascertain the residential status of their employee and his family members from their office records and documents submitted by the employee as their proof of residence. They can also accept Affidavits in absence of valid proof of residence&#8217;.</p>

<div>Government has issued detailed guidelines for issuance of individual plastic cards to CGHS beneficiaries vide Office Memorandum No. S.11012/3/2011-CGHS(P) dated 29th December, 2011 which is available at the CGHS website http://msotransparent.nic.in</div>
<div></div>
<div>Total 1,32,909 applications were received for making of CGHS cards in last one year. It is a continuous process.It has since been streamlined and the cards are now sent to the applicants timely by speed post at their home address. There is no pendency in issuance of CGHS cards.</div>

A copy of Circular received from CGHS Bangalore vide their letter No: Plastic Cards/CGHS/Admn/2521 dtd 7th June 2012 regarding Issue of Individual / Plastic Cards is enclosed. A copy of guidelines on issue of Individual Plastic Cards to each CGHS Beneficiary (serving and retired) received from the Ministry of Health and Family Welfare NewDelhi is also enclosed.</div>

<p><a name="more"></a></p>
<div align="justify">The procedures to be followed in CGHS Bangalore are as follows:</div>
<div align="justify">1) The new procedure shall be effective from 1st August 2012.</div>
<div align="justify"></div>
<div align="justify">2) All blank CGHS Cards Issued to Central Government Departments should be surrendered to Main Office positively before 31st July 2012 for onward transmission to the O/o the AddI Director, Kendriya Sadan, Koramangala.</div>
<div align="justify"></div>
<div align="justify">3) Proof of residence should accompany alongwith the Requisition form.</div>
<div align="justify"></div>
<div align="justify">4) Heads of the PAO’s should take personal responsibility to ensure the correctness of the particulars furnished by the applicant before countersigning the requisition Form &#8220;A&#8221; as the sponsoring authority. Contact Mobile Number or landline number should Invariably be given by all CGHS beneficiaries in the form.</div>

<div align="justify">5) Present Pay and Grade Pay of serving employees should be mentioned separately.</div>
<div align="justify"></div>
<div align="justify">6) All Central Government Department should mention the exact number of their employees covered under CGHS as on 31/7/2012.</div>

										<content:encoded><![CDATA[<p><strong>Issue of Individual CGHS Plastic Cards to all (Retired and Existing) beneficiaries</strong></p>
<p>It has been decided by the office of PCDA Bangalore, to Issue Individual Plastic Card to all CGHS beneficiaries from 1st August 2012 by CGHS Koramangala, Bangalore. In this connection, the guidelines on Issue of Individual Plastic Cards to each CGHS beneficiary (serving and retired) are revised as follows:</p>
<p>1) CGHS Cards shall be issued only to the eligible Central Government Employees whose place of residence is situated within the coverage area of CGHS.</p>
<p>2) Requisition for CGHS shall be prepared in duplicate in Form “A”.</p>
<p>3) Requisitions for CGHS Cards shall be accompanied by two copies of recent 3X5 cm size individual photographs of all the family members of the government employee. One set of the photograph shall be signed on the back by the concerned beneficiary &amp; enclosed with the application form.</p>
<p>4) Requisitions shall be sent along with two copies of the challan as on Form ‘C’ duly filled in.</p>
<p>5) On the occurrence of death, CGHS Cards issued to the government employee shall be withdrawn for cancellation.</p>
<p>6) In case of change in entitlement for CGHS facilities, the Government employee shall enclose the CGHS Card(s) with the application for Issuing new card(s) with the revised entitlement.</p>
<p>7) In case of mutilation, the mutilated card shall be enclosed with the application along with the challan in token of payment of the prescribed charges for issuing a new card.</p>
<p>8) Duplicate Cards may be issued on payment of prescribed fee with details of lost / misplaced cards.</p>
<p>9) The plastic cards issued by CGHS shall be valid for a period of five years from the date of issue. The validity period shall also be indicated on the card.</p>
