<p>Corporate hospitals across the country have threatened to stop cashless treatment to lakhs of Central Government employees from January 1. This will impact about 45 lakh employees of the Central Government and public sector agencies who are benefiting from the insurance scheme.<br />
Protesting against “unviable” rates being given to them under the Central Government Health Scheme (CGHS), the Association of Healthcare Providers (India) (AHPI) has said that it will give three months’ time to the Government to revise the rates.</p>
<p>“After that we will stop all cashless services at our hospitals. We will, however, continue to provide services on paying cash (at the hospital rates). They can seek reimbursement later,’’ said Alex Thomas, an AHPI leader.</p>
<p><strong>PRICE CORRECTION</strong></p>
<p>Representatives of about 100 corporate hospitals held a meeting here on Saturday to discuss the challenges they face with regard to CGHS cases. AHPI said the Government should come out with a scientific method to arrive at proper packages for various treatments.</p>
<p>“They have not revised the tariff for various procedures for years, while the cost of operations has gone up significantly,” said Bhaskara Rao, President of the Andhra Pradesh Speciality Hospitals Association and Chief Executive Officer of Krishna Institute of Medical Sciences.<br />
The Association will submit a memorandum to the Centre through CGHS authorities, demanding a hike in tariff for several packages.</p>
<p>Deviprasad Shetty, Chairman of Narayana Hrudayalaya, said hospitals are not being paid on time, and that the payments are below the actual cost (of procedures).</p>
<p>When asked about the alleged malpractices and inflation of bills by corporate hospitals, both Thomas and Rao said that all such violations should be probed into.</p>

<p>Sub: <strong>Suspension of empanelment of &#8216;Saket City Hospital, New Delhi&#8217; from the list of Hospitals / Centres empanelled under CGHS &#8211; reg.</strong></p>
<p>The undersigned is directed to state that CGHS has empanelled private hospitals in various <a href="http://centralgovernmentnews.com/category/cghs/" target="_blank">CGHS</a> covered cities to provide inpatient healthcare facilities to its beneficiaries. Empanelment of the private hospitals is done under a contractual agreement between the Government of India and the private hospital on mutually agreed terms and conditions.</p>
<p>2. This Ministry has received a number of complaints against Saket City Hospital (formerly- Gujar Mal Modi Hospital &amp; Research Centre), Saket, New Delhi regarding deficiency in services ranging from denial of credit facilities and over charging to illtreatment, harassment and rude behavior by staff/doctors to CGHS beneficiaries. The issues raised by the beneficiaries have been viewed seriously by this Ministry as it tantamount to violation of the terms and conditions of empanelment as laid down in the &#8216;Memorandum of Agreement&#8217;.</p>
<p>3. A &#8216;Show Cause Notice&#8217; was issued to Saket City Hospital seeking clarifications on the grievances raised by the beneficiaries. However, the reply received from Saket City Hospital has not been found satisfactory.</p>
<p>4. It has, therefore been decided to suspend the CGHS empanelment of Saket City Hospital, New Delhi with immediate effect, for a period of one year or till further orders whichever is earlier. The hospital shall no longer be a part of the list of empanelled hospitals/centers for any purposes under CGHS. Patients already admitted prior to the issue of this OM, shall be provided treatment and discharged within seven days from the date of issue of this OM. However, in respect of the patients who remain admitted beyond seven days, their bills would be accepted only on submission of the proper justification for stay.</p>
<p>5. This OM shall be effective from the date of its issue.</p>

Subject: <strong>Recognition of Aayush NRI LEPL Health Care Pvt. Ltd., Vijayawada, Andhra Pradesh for treatment of Central Government employees under CS (MA) Rules, 1944.</strong></p>
<p>The undersigned is directed to say that the proposal received for recognition of Aayush NRI LEPL Health Care Pvt. Ltd., Vijayawada (Andhra Pradesh) for treatment of Central Government  Employees and their family members under CS(MA) Rules, 1944 was under consideration.</p>
<p>2.         In view of the hardship faced by CS (MA) beneficiaries for their own treatment and the treatment of their family members at Vijayawada, (Andhra Pradesh) the matter has been examined in the Ministry and it has been decided to empanel Aayush NRI LEPL Health Care Pvt. Ltd., Vijayawada (Andhra Pradesh) under Central Services (Medical Attendance) Rules, 1944.</p>
<p>3.         The Schedule of charges for the treatment of Central Government Employees and the members of their family under the CS (MA) Rules, 1944, will be the rates fixed for CGHS, Hyderabad. The approved rates are available on the website of CGHS (www.mohfw.nic.in//cghs.html) and may be downloaded/printed.</p>
<p>4.         The undersigned is further directed to clarify as under:-</p>
<blockquote><p>(a)       “Package Rate” shall mean and include lump sum cost of in-patient treatment//day care/diagnostic procedure for which a CS(MA) beneficiary has been permitted by the competent authority or for treatment under emergency from the time of admission to the time of discharge, including (but not limited to)-(i) Registration charges, (ii) Admission charges, (iii) Accommodation charges including patient’s diet, (iv) Operation charges, (v) Injection charges, (vi) Dressing charges, (vii) Doctor/consultant visit charges, (viii) ICU/ICCU charges, (ix) Monitoring charges, (x) Transfusion charges, (xi) Anesthesia charges, (xii) Operation theatre charges, (xiii) Procedural charges / Surgeon’s fee, (xiv) Cost of surgical disposables and all sundries used during hospitalization, (xv) Cost of medicines, (xvi) Related routine and essential investigations, (xvii) Physiotherapy charges etc, (xviii) Nursing care and charges for its services.</p>
<p>(b)       Cost of Implant is reimbursable in addition to package rates as per CGHS ceiling rates for implants or as per actual, in case there is no CGHS prescribed ceiling rates.</p>
<p>(c)       Treatment charges for new born baby are separately reimbursable in addition to delivery charges for mother.</p>
<p>(d)       Aayush NRI LEPL Health Care Pvt. Ltd., Vijayawada (Andhra Pradesh) shall not charge more than the package rates fixed for CGHS, Hyderabad.</p>
<p>(e)       Expenses on toiletries, cosmetics, telephone bills etc. are not reimbursable and are not included in package rates.</p></blockquote>
<p>5.         Package rates envisage duration of indoor treatment as follows:</p>
<blockquote><p>Upto 12 days: for Specialzied (Super Specialities) treatment</p>
<p>Upto 7 days: for other Major Surgeries</p>
<p>Upto 3 days: for Laparoscopic surgeries/normal Deliveries</p>
<p>1 day:             for day care/Minor (OPD) surgeries.</p></blockquote>
<p>No additional charge on account of extended period of stay shall be allowed if that extension is due to infection on the consequences of surgical procedure or due to any improper procedure and is not justified.</p>
<p>In case, there are no CGHS prescribed rates for any test/procedure, then AIIMS rates are applicable. If there are no AIIMS rates, then reimbursement is to be arrived at by calculating admissible amount item-wise (e.g. room rent, investigations, cost of medicines, procedure charges etc) as per approved rates/actually, in case of investigations.</p>
<p>6.         (a) CS (MA) beneficiaries are entitled to facilities of private, semi-private or general ward depending on their basic pay. The entitlement is as follows:-</p>

<p>(b) The package rates given in rate list are for semi-private ward.</p>
<p>(c) The package rates prescribed are for semi-private ward. If the beneficiary is entitled for general ward there will be a decrease of 10% in the rates; for private ward entitlement there will be an increase of 15%. However, the rates shall be same for investigation irrespective of entitlement, whether the patient is admitted or not and the test, per-se, does not require admission.</p>
<p>7.         The hospital shall charge from the beneficiary as per the CGHS prescribed rates or its own rate list whichever is lower.</p>
<p>8.</p>
<p>&nbsp;</p>
<blockquote><p>(a) The maximum room rent admissible for different categories would be:</p></blockquote>
<blockquote><p>General ward                                    Rs. 1000/- per day<br />
Semi-private ward                Rs. 2000/- per day<br />
Private ward                          Rs. 3000/- per day<br />
Day care (6 to 8 Hrs.)          Rs. 500/- (same for all categories)</p></blockquote>
<p>&nbsp;</p>
<blockquote><p>(b) Room rent mentioned above at (a) above is applicable only for treatment procedures for which there is no CGHS prescribed package rate.</p>
<p>Room rent will include charges for occupation of bed, diet for the patient, charges for water and electricity supply, linen charges, nursing charges and routing up keeping.</p>
<p>(c) During the treatment in ICCU/ICU, no separate room rent will be admissible.</p>
<p>(d) Private ward is defined as a hospital room where single patient is accommodated and which has an attached toilet (lavatory and bath). The room should have furnishings like wardrobe, dressing table, bed-side table, sofa set, etc. as well as bed for attendant. The room has to be air-conditioned.</p>
<p>(e) Semi Private ward is defined as a hospital room where two to three patients are accommodated and which has attached toilet facilities and necessary furnishings.</p>
<p>(f) General ward is defined as hall that accommodates four to ten patients.</p>
<p>(g) Normally the treatment in higher category of accommodation than the entitled category is not permissible. However, in case of an emergency when the entitled category accommodation is not available, admission in the immediate higher category may be allowed till the entitled category accommodation becomes available. However, if a particular hospital does not have the ward as per entitlement of beneficiary, then the hospital can only bill as per entitlement of the beneficiary even though the treatment was given in higher type of ward.</p></blockquote>
<p>If, on the request of the beneficiary, treatment is provided in a higher category of ward, then the expenditure over and above entitlement will have to be borne by the beneficiary.</p>
<p>9.         In case of non-emergencies, the beneficiary shall have the option of availing specific treatment/investigation from any of the above mention hospitals of his/her choice (provide the hospital is recognized for that treatment procedure/test), after the specific treatment/investigation has been advised by Authorised Medical Attendant and on production of valid ID card and permission letter from his/her concerned Ministry/Department.</p>
<p>10.       The hospital shall honour permission letter issued by competent authority and provide treatment/investigation facilities as specified in the permission letter.</p>
<p>11.       The hospital shall also provide treatment/investigation facilities to the CGHS beneficiaries and their eligible dependent family members at their own rates or rates approved under CS (MA) Rules, whichever is lower. The hospital shall provide treatment to such pensioner CGHS beneficiaries after authentication through verification of valid CGHS Cards.</p>
<p>12.       However, pensioner CGHS beneficiaries would make payment for the medical treatment at approved rates as mentioned above and submit the medical reimbursement claim to the Addl. Director, CGHS through the CMO i/c of the CGHS Wellness Centre, where the CGHS Card of the beneficiary is registered.</p>
<p>13.       In case of emergencies, the beneficiary shall have the option of availing specific treatment/investigation from any of the above mentioned hospitals of his/her choice (provided the hospital is recognized for that treatment procedure/test), on production of valid ID card, issued by competent authority.</p>
<p>14.       During the in-patient treatment of the CS (MA) beneficiary, the Hospital will not ask the beneficiary or his attendant to purchase separately the medicines/sundries/equipment or accessories from outside and will provide the treatment within the package rate, fixed by the CGHS which includes the cost of all the items.</p>
<p>15.       In case of treatment taken in emergency in any non-recognised private hospitals, reimbursement shall be considered by competent authority at CGHS prescribed Package/rates only.</p>
<p>16.       If one or more minor procedures form part of a major treatment procedure, then package charges would be permissible for major procedure and only 50% of charges for minor procedure.</p>
<p>17.       Any legal liability arising out of such services shall be the sole responsibility and shall be dealt with by the concerned empanelled hospital. Services will be provided by the Hospital as per the terms given above.</p>
<p>18.       Ministry of Health &amp; Family Welfare reserves the right to withdraw/cancel the above recognition without assigning any reason.</p>
<p>19.       The order takes effect from the date of issue of the O.M.</p>
<p>20.       The authorities of Aayush NRI LEPL Health Care Pvt. Ltd., Vijayawada (Andhra Pradesh) will have to enter into an agreement with the Government of India to the effect that the Hospital will charge from the Central Government employees at the rates fixed by the Government and they will have to sign a Memorandum of Understanding (MOU) (2 copies enclosed only for Hospital) within a period of 3 months from the date of issue of the above mentioned OM failing which the Hospital will be derecognized. Subject to above, the Hospital can start treating Central Government employees covered under CS (MA) Rules, 1944.</p>

<p>Sub: <strong>Notification of fresh empanelment of Private Hospitals &amp; Diagnostic Centers under continuous empanelment scheme of CGHS, Meerut.</strong></p>
<p>In reference to MOHFW OM No. S.11011/23/2009-CGHS D II / Hospital Cell (part-IX) dated 14-02-2013 and this Directorate letter No. S11045/23/2013/CGHS D-II (HEC)/CGHS (P)/(Pt) dated 15-07-2013 for continuous fresh empanelment of Private Hospitals &amp; Diagnostic Centers under CGHS Meerut, a committee was formed with two senior most CMOs headed by undersigned including Consultant as Special Invitee. Inspection for authentication of documents submitted was carried out by Transparency officer, CGHS Meerut, Office Superintendent CGHS Meerut &amp; Sr. Consultant CGHS Meerut as Special Invitee. The qualified list of Hospitals &amp; Diagnostic Centers is according to category and purpose attached as Annexure &#8220;A&#8221; with this OM (on the recommendation of Hospital Committee) for One Year or Fresh Empanelment whichever is earlier.</p>
<p>1. The Hospitals &amp; Diagnostic Centers who have qualified to be empanelment under CGHS, Meerut have submitted the Draft MOA with CGHS along with performance Bank Guarantee are approved for empanelment of Private Hospitals &amp; Diagnostic Centers under <a href="http://centralgovernmentnews.com/category/cghs/">CGHS</a>, Meerut with effect from date of notification of the OM. As fresh empanelment mentioned at Sr. No 10, 11, 17 &amp; 18 and from Sr.No 1 to 4 have added additional facilities after giving an undertaking on Rs.100.00 rupees Non Judicial Stamp paper and remaining are old empanelled Hospitals &amp; Diagnostic Centers.</p>
<p>2. The empanelment shall be for a period of one year from the date of notification or fill new empanelment process whichever is earlier.</p>

<p>The issue of laying down uniform guidelines to be adopted for Liver Transplant Surgery of Railway Health beneficiaries has been engaging attention of Ministry of <a href="http://centralgovernmentnews.com/category/railways/">Railways</a> for sometime. After careful consideration of the matter, it has been decided to stipulate the under mentioned guidelines for adoption in all cases of Liver Transplantation :-</p>
<p>1. Selection Criteria<br />
A. Indications<br />
(i). Adult Liver diseases</p>

<p><strong>3. Centres Approved for Liver Transplantation Surgery</strong><br />
Liver Transplant Surgery shall be allowed only in Government Hospitals/ Pvt. Hospitals, which are registered under the Transplantation of Human Organs Act, 1994, as amended from time to time.</p>
<p><strong>4. Documents required</strong> to be submitted for consideration of permission for liver transplant surgery</p>
<blockquote><p>(i) Recommendation by Govt. / RIy. Gastroenterolgist/GI Surgeon.<br />
(ii) CT/ MRI Liver report.<br />
(iii) Etiology evaluation report.<br />
(iv) Histopathological report, wherever available.<br />
(v) Current Child Pugh/MELD score report.<br />
(vi) Other relevant document.</p></blockquote>
<p><strong>5. Package Charges for Liver Transplantation Surgery</strong></p>
<p>a) Package rate for Liver Transplantation Surgery involving live Liver donor shall be &#8211; Rs. 14,00,000/- (Rupees Fourteen Lakhs only). This would include Rs.2,50,000/- (Rupees Two Lakhs Fifty Thousand only) for pre-transplant evaluation of the donor and the recipient and Rs. 11,50,000/- (Rupees Eleven Lakhs Fifty Thousand only) for transplant surgery.</p>
<p>b) Package rate for Liver Transplant Surgery involving deceased donor shall be Rs.11,00,000/- (Rupees Eleven Lakhs only).</p>
<p>The above package includes the cost of consumables during the organ retrieval and the cost of preservative solution etc. The package charges also include the following:</p>
<blockquote><p>(i) 30 days stay of the recipient and 15 days for the donor starting one day prior to the transplant surgery.</p>
<p>(ii) Charges for Medical and Surgical Consumables, surgical and procedure charges, Operation theatre charges, Anaesthesia Charges, Pharmacy charges etc.</p>
<p>(iii) Investigations and in-house doctor consultation for both donor and recipient during the above period of stay.</p>
<p>(iv) All post operative investigations and procedures during the above mentioned period.</p></blockquote>
<p>C) The package shall exclude the following :-<br />
Charges for drugs like Basiliximab/Daclizumab, HBIG, and peg interferon.Cross Matching charges for Blood and Blood products.</p>
<p>d) (i) The extra stay if any may be sanctioned/reimbursed after justification by the trcating specialists for the reason of additional stay and only as per Railway RMA/RELHS guidelines.</p>
<p>(ii) The drugs mentioned above would be reimbursed as per <a href="http://centralgovernmentnews.com/category/cghs/">CGHS</a> rates or actual whichever is lower.</p>
<p><strong>6. Reimbursement Criteria:</strong><br />
As Liver Transplant Surgery is a planned surgery and, therefore, prior permission has to be obtained before the surgery is undertaken. However, if for some reason it is done in emergency to save the life of the patient, the medical board shall consider the case referred to it for recommending grant of ex-post-facto permission on a case to case basis.</p>
<p><strong>7. Procedure for Sanction:</strong><br />
CMD of the zone shall nominate Medical Board comprising of a CMS/MD as the Chairman and two specialists each from Gastroenterology and GI Surgery as members which will recommend for Liver Transplantation. The proposal for financial sanction would be considered in consultation with finance of the zone and approval of General Manager before forwarding the same to Railway Board for sanction.</p>
<p><strong>8. Other terms &amp; conditions</strong><br />
For payment of advance as per instructions laid down by this office from time to time will remain unchanged.</p>
<p>9. This issues with the concurrence of the Finance Dte. of Ministry of Railways.</p>
<p>10. These guidelines shall come into effect from the date of issue of this letter.</p>

<p>Sub: <strong>Merger of 19 Postal Dispensaries presently functioning in 12 CGHS covered cities with CGHS.</strong></p>
<p>In pursuance of the decision taken by the Government on recommendations of <a href="http://centralgovernmentnews.com/category/6cpc/" target="_blank">Sixth Pay Commission</a>, the followIng 19 (nineteen) Postal Dispensaries presently functioning in 12 (twelve) CGHS covered cities. i.e., Ahmadabad (3), Bhopal (1), Bhubaneswar (1), Dehradun (1), Guwahati (2), Jaipur (2), Jammu (1), Jabalpur(1), Lucknow (3), Pune (2), Ranchi (1) and Shillong (1) are hereby merged with the Central Government Health Scheme (CGHS).</p>
<p>2. All serving employees and pensioners of Department of Posts (DoP) and Department of Telecom (DoT) who are residing/settled in the above 12 cities and are beneficiaries of the 19 PostaI Dispensaries shall now be covered under CGHS and the Postal Dispensaries shall be rechristened as CGHS Wellness Centres. <a href="http://centralgovernmentnews.com/category/cghs/" target="_blank">CGHS</a> membership to the pensioners will be confined to those who are residing / settled in these 12 cities only.</p>
<p>3. In so far as the existing facilities and manpower in position In these 19 Postal Dispensaries are concerned, the merger shall be effective as per the following terms and conditions:</p>
<blockquote><p>A. All serving employees and pensioners of Department of Post (DoP) and Department of Telecom (DoT) shall have to abide by the CGHS rules and guidelines to become a member of the Scheme. They shall have to pay the requisite contribution as per the prevailing rates prescribed by the Ministry of Health and Family Welfare/CGHS. DoP and DoT will cake necessary action to inform their employees and pensioners in this regard.</p>
<p>B. All existing facilities and infrastructure like buildings, furniture and fixtures, equipment etc. will be taken over by CGHS on ‘as is where is’ basis. The Department of Post shall handover the possession of the Postal Dispensaries accommodation to the Department of Health and Family Welfare /CGHS at a token rent of Re.1/- per annum. In the case of rented accommodations, CGHS will pay the rent from the date of taking over of the dispensaries.</p>
<p>C. All doctors of GDMO sub-cadre of CHS working in the above 19 dispensaries will be taken on roll of CGHS and they shall be placed under the administrative control of Department of Health and Family Welfare/CGHS for all purposes.</p>
<p>D. All employees (technical/non-technical staff) along with the work allocated and posts they are currently holding in these 19 Postal Dispensaries shall be taken over by CGHS. Their seniority and other condition of service in CGHS shall be governed by the relevant instructions and guidelines issued by DoPT from time to time.</p>
<p>E. All expenditure relating to these dispensaries including medicines, hospitalisation and other reimbursable expenses (of pensioners), salaries and other allowances to the Postal dispensary employees as a result of merger of these dispensaries shall be borne by CGHS from its own resources.</p>
<p>F. Local Committees shall be constituted in the respective cities with representatives from both CGHS and Postal dispensaries to resolve all staffing and other local issues arising on account of the merger in consultation with nodal Ministries.</p></blockquote>
<p>4. These Orders shall be effective from 1st August, 2013.</p>
<p>5. This issues with the concurrence of Ministry of Finance, Department of Expenditure’s vide I.D No. 18(3)/E.V/2008 dated 06.03.2013.</p>

<p>Central Government Health Scheme &#8211; New revised package rates for Coronary Angioplasty under CGHS:</p>
<p><strong><em>New Rates for Undergoing Angioplasty</em></strong></p>
<p>The Government has revised the CGHS package rates for coronary angioplasty and angioplasty with balloon w.e.f. 7/2/2013 as per the following details:</p>
<p>Coronary Angioplasty and Angioplasty with Balloon</p>
<p><strong>Coronary Angioplasty &#8211;  Rs. 50,000/- </strong><br />
<strong>Coronary Angioplasty with Balloon &#8211;  Rs. 55,000/-</strong></p>
<p>All the empanelled private hospitals have not discontinued to provide angioplasty treatment to CGHS beneficiaries. However, ‘3’ hospitals in Delhi &amp; NCR have  filed Writ Petitions in Hon’ble High Court of Delhi against the revised rates.</p>
<p>Meanwhile, the continuous empanelment scheme has been revived on 14th February, 2013 to empanel more number of eligible private hospitals under CGHS to provide inpatient healthcare facilities to CGHS beneficiaries.</p>
<p>This information was given by the Minister for Health &amp; Family Welfare ShriGhulamNabi Azad in written reply to a question in the RajyaSabha today.</p>

<p>Subject:<strong> Revival of ‘Continuous Empanelment Scheme’, for private hospitals and diagnostic centres under CGHS.</strong></p>
<p>With a view to ensuring comprehensive health care to CGHS beneficiaries, CGHS has been, apart from the Government Hospitals, empanelling private hospitals and diagnostic centres by floating tenders periodically. The latest tender process in this context commenced in the second half of 2009. Even the already empanelled hospitals (including dental clinics and eye centres) / diagnostic centres under CGHS were required to submit the bids under this tender process. Through this process, package rates for different treatment procedures and investigations were notified in 2010. As a part of this process, certain hospitals and diagnostic centres which accepted these rates and also signed MOAs with CGHS were notified in CGHS covered cities. Subsequently, with a view to empanel more hospitals and diagnostic centers , continuous empanelment scheme was initiated in December 2010 and was in operation till July 2011 in some cities &amp; till 26th December 2011 in some other cities.</p>
<p>2. However, it has been noticed that the number of hospitals and diagnostic centres notified as empanelled under CGHS is not adequate to provide a satisfactory level of health care to CGHS beneficiaries in all areas of CGHS covered cities.</p>
<p>3. Accordingly, it has become necessary to revive ‘Continuous Empanelment Scheme’, to empanel more hospitals and diagnostic centres to take care of existing inadequacy. In this background, it has been decided to invite private hospitals and diagnostic centers to come forward and apply for empanelment under CGHS if, they are willing to accept the rates notified by CGHS in 2010 &amp; as per OM No.Misc.1002/2006/CGHS(R&amp;H)/CGHS (P) dated 6/2/2013 and fulfill the same terms and conditions as notified earlier under ‘Continuous Empanelment Scheme’ except for the following modifications :-</p>
<p>a) Hospitals / Diagnostic centres shall be empanelled for all facilities in all specialties available in the hospitals including those listed under super specialties. Existing hospitals already on panel of CGHS shall not be allowed to add selective specialties/super specialties and will have to offer all available facilities (Undertaking shall be submitted on Rs.100/- non-Judicial Stamp paper attested by Notary Public by hospitals already on panel of CGHS that such hospital shall abide by the terms and conditions of empanelment for the additional facilities and would provide facilities to CGHS beneficiaries in all available specialties/Super specialties (list all the facilities available ) in addition to the agreement signed earlier by such hospitals). The empanelled hospitals will not be permitted to add new specialties or discontinue some specialties subsequently without the approval of the Government.</p>
<p>b) Application under Super Specialty category or change of category to Super Specialty from already empanelled and other eligible hospitals will be considered only if all facilities available in the hospital are offered as per applicable CGHS rates..</p>
<p>c) The scrutiny of the applications and finalization of the lists of eligible hospitals and diagnostic centres of a particular city shall be done by a committee under the chairmanship of AD/JD, CGHS of concerned city with two senior most CMOs of that city as members.</p>
<p>d) Addl. Director / Joint Director of concerned CGHS city may inform the eligible hospitals / diagnostic centres to submit the letters of acceptance of the terms and conditions of the empanelment process. ADs/JDs shall send the details of eligible hospitals / diagnostic centres to Director, CGHS after signing MOA with eligible hospitals/ diagnostic centres and obtaining Performance Bank Guarantee so that the eligible hospitals /diagnostic centres shall be notified by Ministry of Health &amp; Family Welfare as empanelled hospitals / diagnostic centres under CGHS.</p>
<p>4. Empanelment shall be for a period of one year from the date of notification or till new empanelment process , whichever is earlier. All the empanelled hospitals / diagnostic centres shall however, have to participate in the new empanelment process, as and when initiated inorder to continue their empanelment under CGHS.</p>
<p>5. The continuous empanelment scheme would be in force till next empanelment.</p>
<p>6. Further, the undersigned is directed to refer to the Office Memorandum of even number dated 15.12.2012 and to state that it has been decided to extend the validity of empanelment of presently empanelled hospitals, diagnostic laboratories and imaging Centers on the same terms and conditions under which they were empanelled for one year or till next empanelment, whichever is earlier.</p>
<p>7. The application form for the continuous empanelment scheme can be downloaded from the website of CGHS, www.mohfw.nic.in\cghsnew\index.asp.</p>

<p>Subject : <strong>Removal / Suspension of Escorts Heart Institute &amp; Research Centre, Max Super Specialty Hospital and Max Devki Heart &amp; Vascular Institute, New Delhi &#8211; regarding</strong></p>
<p>With reference to the above mentioned matter, the undersigned is directed to draw attention to the Office Memorandum No S.11011/23/2009/CGHS DII / Hospital Cell (part I) dated 7.10.2010 vide which Escorts Heart Institute &amp; Research Centre, Max Super Specialty Hospital and Max Devki Heart &amp; Vascular Institute, New Delhi were empanelled under CGHS, Delhi and to state that Escorts Heart Institute &amp; Research Centre and Max Super Specialty Hospital, New Delhi have conveyed their unwillingness to continue their empanelment under CGHS. It is also stated that Max Devki Heart &amp; Vascular Institute, New Delhi informed that the hospital shall not provide credit facility to CGHS beneficiaries. The matter has been examined by this Ministry and it has been decided that Escorts Heart Institute &amp; Research Centre, Max Super Specialty Hospital, New Delhi shall stand removed from the list of empanelled hospitals under CGHS, Delhi with immediate effect.</p>
<p>2. It is further stated that the empanelment of Max Devki Heart &amp; Vascular Institute, New Delhi is suspended from the list of empanelted hospitals under CGHS with immediate effect till further orders.</p>
