<h3 class="wp-block-heading"><strong>Subject: Validity of CGHS Card in all CGHS Centres</strong></h3>



<p>The undersigned is directed to draw attention to the Order No Misc 6024/2007/ CGHS(HQ)/ CGHS(P) dated the 17th December 2012 and to reiterate that CGHS Card is valid at any CGHS Wellness Centre in the Country for availing CGHS benefits as per the details given under:</p>



<ul class="wp-block-list">
<li>i) CGHS Card is valid in any Wellness Centre in India and no additional document / attachment is required.</li>



<li>ii) Medicines shall be issued for upto to &#8216;3&#8217; months, in case of beneficiaries undergoing treatment for chronic illnesses.</li>



<li>iii) Referral/ endorsement for treatment from private HCOs empanelled under CGHS shall be issued from any CGHS Wellness centre.</li>
</ul>



<p>Additional Directors, CGHS are advised to ensure that these guidelines are complied with by CMOs in Charge and other staff of CGHS Wellness Centres.</p>



<p>The contents of this order are to be brought to the notice of Members of the Local Advisory Committees/ Zonal Advisory Committee.</p>

<h3 class="wp-block-heading">Subject: Issue of life time CGHS Card to BSNL pensioners.</h3>



<p>The matter regarding issue of life time CGHS card to BSNL pensioners who had deposited CGHS contribution before switching over to the health scheme of BSNL and are desirous of switching over again to CGHS was under examination in the Ministry for quite some time past.</p>



<p>2. It has now been decided that BSNL pensioners who draw their pension from Central Civil Estimates are entitled for CGHS facilities. In case of their switch over again to CGHS from the health scheme of BSNL, they are required to pay subscription at the prevalent rates only for the balance period for life time membership i.e. the number of years for which subscription was already paid by them to CGHS before being switched over to BSNL health scheme, will be deducted from the ten-year subscription period. However, the option of payment of subscription at prevalent rate on annual basis will continue to be available to them for the balance period and the life time CGHS card will be issued on completion of payment of subscription for the total period of ten years.</p>

<h3 class="wp-block-heading" id="subject-storage-of-cghs-card-on-digilocker-platform-for-its-access-and-use-for-cghs-services">Subject: Storage of CGHS Card on DigiLocker platform for its access and use for CGHS Services</h3>



<p>Central Government Health Scheme, in alignment with its vision of being the first choice in providing quality healthcare services and ensuring holistic well-being across clients entire life span, has found it imperative to adopt new technology and innovations in the delivery of quality healthcare.</p>



<p>In continuation of the these measures, it has now been decided to provide an option to store CGHS Card in DigiLocker. DigiLocker is a secure cloud based platform for storage, sharing and verification of government issued documents &amp; certificates. It is available as a mobile application for both Android and iOS devices. Through DigiLocker Platform a CGHS beneficiary would be able to download his/her CGHS card on the mobile device.</p>



<p><strong>The downloaded CGHS card shall be equivalent to the original CGHS card as per the&nbsp;</strong><strong>provisions of Rule 9A of the Information Technology Rules, 2016, notified on February&nbsp;</strong><strong>8, 2017 via G.S.R. 714(E).</strong></p>



<p>CGHS beneficiaries would need to download the DigiLocker App and login using their Aadhar and mobile number. A beneficiary can access only his own CGHS card at a time. In case his mobile number linked with his Aadhar is also linked with his dependent(s) Aadhar, then he will be able to access his dependents CGHS card too, only after fresh login to DigiLocker.</p>



<p>The steps involved for registering on DigiLocker and Application and Downloading the CGHS cards may be seen at the Annexure attached.</p>

<h4 class="wp-block-heading">Subject: Facility for downloading of Index Card and Validity of CGHS Card &#8211; Regarding.</h4>



<p>It has been the endeavor of CGHS to provide more on-line facilities for its beneficiaries, especially during <a href="https://centralgovernmentnews.com/tag/Covid-19/" target="_blank" rel="noreferrer noopener">COVID</a> times. As a step further in this direction, the facility to download and print the Index Card is now available to the CGHS beneficiaries. On verification of the CGHS card application by the Additional Director of the CGHS city/ headquarters, an SMS would be sent to the registered mobile number of the main cardholder, following which the Index Card would be downloadable by the main cardholder and his dependents. The beneficiary will no longer be required to visit the Office of the Additional Director of the city/ headquarters at Delhi to collect the paper Index Card. The beneficiary can use the downloaded index Card (e-index Card)) to avail all CGHS facilities in <a href="https://centralgovernmentnews.com/category/cghs/" target="_blank" rel="noreferrer noopener">CGHS</a> units as well as at empanelled hospitals and diagnostic centers.</p>



<h4 class="wp-block-heading">How to download the Index Card:</h4>



<p>On receiving the SMS regarding verification of card application, the beneficiary would be required to go to “Beneficiary login” at www.cghs.nic.in and log in using his Beneficiary ID and password. An OTP will be sent to the registered mobile number as well as the email of the beneficiary for authentication, following which the beneficiary would be able to download the e-index Card.</p>



<h4 class="wp-block-heading">Validity of CGHS Plastic Cards:</h4>



<p>Earlier, CGHS cards of serving beneficiaries and their dependents were valid for 05 years from the date of being printed. This often resulted in differing card expiry date of main card holder and his dependents, for instance, in case of addition of a new dependent.</p>



<p>To remove this anomaly, henceforth, in case of all new/ renewed serving <strong>CGHS cards,</strong> the validity period of the Plastic Card, e-index Card, as well as “Print your own Card” will be the same for the main cardholder and all their dependents i.e.,05 years from the date when the department has approved the application for the plastic card of the main cardholder’ This will be irrespective of the date when their card is printed. ln addition, the e-index Card and &#8220;Print your own Card&#8221; facility would be available during this 05 year validity period only.</p>



<p>The existing plastic cards will however continue till their 05 year validify period ends.</p>



<p>ln case of pensioner cards (except whole life card), the e-index Card / plastic card/ &#8220;Print your own card&#8221; validity and download facility would be from the <strong>date of approval on the application for plastic card by Additional Director of the city/ Headquarter in Delhi, till the period covered by the subscription deposited by the pensioner.</strong></p>



<p>The plastic card printing agency will henceforth print CGHS cards of main card holder and dependents with the same “Valid upto” date, as captured in the database of the main CGHS card holder (exception being whole life pensioner cards where “whole life” is to be printed in place of”valid upto” date).</p>



<p>Beneficiaries would be able to apply for renewal of plastic cards 03 months from expiry date of existing card. The new e-Index Card valid for 5 years from date of approval by department/ CGHS as the case may be, would be available for downloading after SMS confirmation from CGHS.</p>



<p>It is reiterated that in case any of the beneficiaries [main cardholder or dependent) becoming ineligible for availing CGHS facilities, it shall be the responsibility of the main cardholder to immediately inform the Additional Director /CMO l/C either through the department (in case of serving beneficiaries) or directly (please see at www.cghs.nic.in FAQs at serial numbers 1 and 4 to 9 for details). It is also reiterated that in the event of the main card holder’s failure to inform CGHS about a change in the eligibility status, then CGHS facility to the beneficiary and his dependents is liable to be withdrawn in addition to any other action as deemed fit by CGHS/ appropriate authority (including disciplinary proceedings against serving beneficiary under CCS (CCA) Rules, 1965). Department will be responsible for retaining and getting deleted from the Database the CGHS cards of its employees transferred to non-CGHs covered areas/ organizations or on resignation/retirement.</p>



<p>The information regarding the new e-lndex card facility would be disseminated to the main card holders through SMS.</p>

<p>Subject:- <strong>Guidelines to implement O.M. dated 13.03.2019 for extension of CGHS facilities to the retired employees of Kendriya Vidyalaya Sangathan &#8211; regarding.</strong></p>



<p>Sir/Madam,</p>



<p>In continuation of KVS Office Memorandum of even no. dated 13.03.2019 vide which the approval of the Competent Authority of KVS for extension of CGHS facility to the retired employees of KVS who were having CGHS card while in service and residing in CGHS covered cities was conveyed. In this connection, the following guidelines may be kept in mind while processing the requests made for making of new CGHS card by the retired employees:-</p>



<p>A. A retired employee of KVS who was having CGHS card while in service and is residing in CGHS covered areas can opt for CGHS facility by submitting his / her application along with relevant documents and CGHS contribution to the concerned <strong>Pension Sanctioning Authority (PSA)</strong> which has released his / her pensionary benefits.  As per 7th CPC, the revised monthly subscriptions, as decided by Ministry of Health &amp; Family Welfare vide their O.M .No. 5.11011/11/2016-CGHS(P)/EHS dated 09.01.2017, to be made by employees/ pensioners for availing CGHS facilities are as under:-</p>

<p>B. On receipt of application from the retired official, the concerned Pension Sanctioning Authority i.e. Regional Office (R.O)/ZIETs/ Kendriya Vidyalaya Sangathan (HQ) will verify the particulars filled in the application form and enclosed documents as required by CGHS Authority i.e. self-attested photocopy of the LPC, PPO, Aadhar Card, CGHS card while in  service, employee contribution as per 7th CPC as applicable to the concerned employee in accordance to his / her pay level at the time of retirement.  Thereafter, PSA will write  a letter for  discontinuation  of payment of <strong>fixed  medical allowance (FMA)</strong> to the Pension Disbursing Authority (PDA) i.e. State Bank of India, Parliament Street, New Delhi. The PSA will forward the application form along with Cheque / draft for the amount  as per the  CGHS rate applicable with reference to residential address  of the  official concerned to the Deputy Commissioner concerned of the Regional Office having jurisdiction over the CGHS city wherein the official is residing.</p>



<p style="text-align:center"><strong>Illustration</strong></p>



<blockquote class="wp-block-quote is-layout-flow wp-block-quote-is-layout-flow"><p>A Deputy Commissioner or Assistant Commissioner or Administrative Officer is retired from Mumbai Region having CGHS card while in service &amp; residing in Lucknow i.e. CGHS covered area. He / she  will submit  his / her application  (along with his employee-contribution as per 7th CPC in favour of, KVS, HQ and documents stated in para B) to the PSA (KVS, HQ). The PSA will verify the application form and forwarded the same to KVS, RO Lucknow along with the Cheque/DD (amount as applicable for CGHS Centre at Lucknow). Since the Deputy Commissioner, KVS RO Lucknow is having jurisdiction over CGHS Centre at Lucknow the application form will be countersigned by the Deputy Commissioner, RO Lucknow and submitted to CGHS Centre at Lucknow along with Cheque/DD and other necessary documents for preparation of CGHS card. The Deputy Commissioner RO Lucknow will also maintain a register depicting the name of the employee and family members, CGHS card no., validity of the card and the details of the pension sanction authority along with particular of Kendriya Vidyalaya or Regional office from where the official is retired. Further, Deputy Commissioner, KVS RO Lucknow, will also keep a copy of CGHS card (Index Card) issued by CGHS. All medical claims in respect of the Deputy Commissioner/ Assistant Commissioner/ Administrative Officer retired from Mumbai, will be reimbursed by Regional Office Lucknow.</p></blockquote>



<p>C. The concerned Deputy Commissioner will forward the application to the CGHS center, along with all necessary documents including Cheque or Demand Draft after countersigning the application form, for issue of CGHS card. If the retired official is residing in the city having CGHS Centre which falls under the jurisdiction of Pension Sanctioning Authority, then the Pension Sanctioning Authority will forward the application to the concerned CGHS Centre. The Deputy Commissioner concerned will countersign the form and send it to the CGHS authority for preparation of CGHS card under intimation to the retired official as well as the Pension Sanctioning Authority.</p>



<p>D. The Deputy Commissioner of the RO, who has forwarded the application form to the CGHS Centre for issue of card, will maintainthe proper records and make necessary entries in CGHS card issuing register which will carry the details of Name of the employee and family members, CGHS card, validity of the card and the details of the Pension Sanctioning Authority along with particulars of Kendriya Vidyalaya/Regional Office/ZIETs from where the official retired.</p>



<p>E. Existing Rates of CGHS yearly contributions payable by KVS, as circulated by Directorate General of Health Services (DGHS), Nirman Bhawan, New delhi vide their Circular No. Z.22025/4/2007-CGHS (Stats. Cell)  dated  02.03.2010 are given below. The concerned  PSA  will calculate KVS share by deducting the employee contribution as mentioned in para &#8216;A&#8217; from yearly contribution as indicated below. The PSA will then forward the total amount (KVS share &amp; employee contribution) to the respective Deputy Commissioner where the retired official is residing.</p>

<p>The undersigned is directed to state that in accordance with the definition of &#8216;family&#8217; mentioned in Ministry of Health Memo. No. F.6(1) 1-54-H, dated the Ist May, 1954 the family of a beneficiary for the purpose of CGHS scheme includes husband/wife of the CGHS card-holder, as the case may be, wholly dependent children or step-children and parents, who are mainly dependent on and residing with the Government Servant. While it is incumbent upon the card issuing. Authorities, i.e. various departments and offices participating in the scheme, to ensure that the names of only genuine and eligible persons are included in the CGHS token cards, it is the responsibility of the employees concerned to apply for a deletion of the name of the dependent from the CGHS card, when the ward is no more entitled to the benefit eligible under the scheme. The failure on the part of a cardholder to get the name of a child deleted from the CGHS token-card when he is no more dependent on him is a good and sufficient reason for initiating disciplinary proceedings against him in terms of the CCS (CCA) Rules, 1965.</p>
<p>All Ministers/Departments of the Government of India are requested kindly to ensure that the above said provisions are complied with fully. These instructions may also be brought to the notice of all employees concerned.</p>
<p><strong>Opting out of the C.G.H. Scheme</strong></p>
<p>Spouse employed outside Central Government and availing Medical Facilities provided by his/her Employer</p>
<p>Central Government Servants covered under Central Govt. Health Scheme and whose spouse is employed in Defence or Railway Services, State Government or Corporations or Bodies financed partly or wholly by the Central or State Government, Local Bodies and private organizations which provide medical facilities to the employees and their family members, can opt out of the CGH Scheme and avail medical facilities so provided by the above mentioned organizations.</p>
<p>It is to be ensured that neither of the two nor their family members avail medical facilities from both the sources at the same time and for this purpose, the concerned Central Government employee shall give an undertaking to the authority issuing the CGHS Card.</p>
<p><strong>Re-admission under CGHS after &#8216;opting out&#8217;</strong></p>
<p>Such Government Servants, who have opted out of CGHS, may apply for readmission and avail the benefit of CGHS in case their spouse dies or resigns or is dismissed from the office/organization, which provided medical facilities.</p>
<p><strong>Availability of &#8216;Opting Out&#8217; Facility</strong></p>
<p>The facility of opting out of CGHS can only be availed twice during the whole service career of the employee. Administrative Ministry/Department shall record the same by making an entry in the employee&#8217;s Service Book.</p>

<p><strong>LIFE TIME CARD :</strong> CGHS Card for Central Government Pensioners &#8211; Contribution as above depending on the Last grade pay at the time of retirement. Choice is with the individual. One can apply for Life time card by paying 10 years contribution together or one can apply for one year, two years etc. by paying for that applicable period.</p>
<p>The Plastic Cards (which are identity cards), would hereinafter be issued with validity till the pensioner beneficiary is entitled for CGHS benefits. In respect of pensioner CGHS beneficiaries, who have paid CGHS contribution for ‘Rest of Life&#8217; CGHS facilities, the Plastic Cards would, hereinafter, be issued for ‘Rest of Life’.</p>
<p>The ownership of CGHS Card can be transferred in the name of spouse on submission of required documents.</p>
<p>No extra payment to be made, in case of life time cards.</p>

<p>Sub: <strong>Renewal of CGHS Plastic Cards – reg.</strong></p>
<p>The undersigned is directed to refer to this Ministry’s O.M No.S.11012/3/2011-CGHS (P) dated 29.12.2011 laying down the guidelines for issue of individual plastic cards to CGHS beneficiaries. CGHS Plastic Cards were introduced in September, 2008 in Delhi NCR and the cards were initially issued with a validity period of 5 years. The CGHS Plastic Cards completing their validity period are due for renewal and accordingly fresh cards with renewed validity period are being issued by the Office of Additional Director, CGHS of the city concerned. With a view to further streamline the process of renewal of CGHS Plastic Cards, it has been decided to issue the following guidelines supplementing the existing instructions on issue of CGHS Plastic Cards:</p>
<p style="text-align: center;"><span style="text-decoration: underline;"><strong>Serving beneficiary</strong></span></p>
<p>a) Application for renewal of CGHS Plastic Cards in the prescribed proforma (Form AA) alongwith requisite documents (current photographs, copy of pay slip and address proof of residence, if changed), should be submitted through their Administrative Office to the Office of Additional Director, CGHS of the respective CGHS city following the same procedure as prescribed for issue of fresh CGHS card.</p>
<p>b) Fresh CGHS Plastic Cards with same beneficiary ID nos. shall be issued with a validity period of 5 years.</p>
<p>c) Applications for renewal of old plastic cards can be made 3 months in advance prior to its expiry.</p>
<p><strong></strong></p>
<p style="text-align: center;"><span style="text-decoration: underline;"><strong>Pensioner beneficiary</strong></span></p>
<p>a) Application for renewal of Pensioners’ CGHS Plastic Cards in the prescribed proforma (Form BB) alongwith requisite documents (current photographs, PPO or LPC, address proof, if changed) should be submitted to the parent CGHS Wefiness Centre where his /her card is registered. He / she can also submit the application to the Office of Additional Director, CGHS of the respective CGHS city for renewal of CGHS cards.</p>
<p>b) Fresh CGHS Plastic Cards with same beneficiary ID nos. shall be issued with validity for lifetime or up to the date for which the contribution has been made by the beneficiary.</p>
<p>c) Applications for renewal of old plastic cards can be made 3 months in advance prior to its expiry. This issues with the approval of Additional Secretary and Director General, CGHS.</p>

While answering in the Parliament to a question regarding above mentioned the topic, the Minister of Health and Family Welfare Shri Ghulam Nabi Azad said that &#8216;Proof of Residence is required for issuing CGHS cards. <p>He also said that CGHS cards in respect of serving employees and their dependent family members are issued on the recommendation of their Administrative Department / Office. It is incumbent upon the Administrative Department / Office to ascertain the residential status of their employee and his family members from their office records and documents submitted by the employee as their proof of residence. They can also accept Affidavits in absence of valid proof of residence&#8217;.</p>

<p style="text-align: left;">Subject : <strong>Notification of fresh empanelment of Private Hospitals &amp; Diagnostic Centers under continuous Empanelment Scheme of CGHS, Chandigarh</strong></p>
<p>In response to Ministry of Health &amp; Family Welfare, Govt. of India, Nirman Bhawan, New Delhi’s O.M No. S.11011/23/2009-CGHS D. IT/Hospital Cell (Part IX) dated 14.02.2013 &amp; letter No.S.11045/23/2013/CGHS D.II (HEC)/CGHS(P)/Pt) dated 15.07.2013 forcontinuous fresh empanelment of private hospitals &amp; diagnostic centers for city of Chandigarh, a committee consisting of two senior most CMOs headed by undersigned and selected the private hospitals &amp; diagnostic centers for Chandigarh. The qualified list of Hospitals &amp; Diagnostic Centers is according to category and purpose attached as Annexure ‘I’ with this O.M.</p>
<p>2. The hospitals &amp; diagnostic centers who have qualified to be empanelled under CGHS, Chandigarh have submitted the Memorandum of Agreement (MOA) with CGHS alongwith Performance Bank Guarantee, acceptance of CGHS Rates etc. are approved for empanelment of private hospitals &amp; diagnostic centers under CGHS Chandigarh.</p>
<p>3. The empanelnent shall be for a period of one year from the date of notification or till new empanelment process which-ever is earlier subject to the condition that Those Hospital/Diagnostic centers/Imagining certers who have obtained entry level pre accreditation from HABH/NABL at the time of submission of application, such hospitals would however have to obtain final accreditation from NABH/NABL within twelve(12) months from the date of issue of this notification and other hospitals/diagnostic centers have applied for NABH/NABL accreditation with six (06) months failing which their empanelment will stand cancelled.</p>
